New Support Application

Consultation appointment

Please contact us by phone or email, or visit the Office.

Prior interviews at the Office (if necessary)

You and office staff meet and discuss the problems or supports required. About once to three times.
★If application is not made (use the UEC Health Care Center, Open student consultation section, local clinic, etc.) Apply
★If application is made

Preparation/submission of a support application form

The applicant prepares an application summarizing consultation details and submitthree pieces of the new support application formto the University (accepted at the Office for Students with Disabilities).

Internal assessment

The UEC holds a meeting to examine the application received. Then, it determines eligibility and support details.
★(if rejected) A written reply is sent from the University and explanation is given by the Office as an agent of the Special Committee.
★(if accepted)

Setup of an support team/organization of support team meeting*

Staff in charge of student support/academic adviser, Academic Administration Office,

Student Services Office, or other faculty staff discuss an actual support and other matters in detail.

A written reply is sent from the University/Consultation about the start of support

Continuous Support Application

Support details are reviewed at least once an academic term.

Handling of information about disabilities

Regarding contents filled in the support application and consultation/interview details at the Office for Students with Disabilities for application, they may be shared among staff of the office or sections concerned if required for examination and provision of support services.
